In order for any of the youth artists in the Show to receive fees, prize money, or payments for their artworks, the Heard Guild must first have both of these forms properly signed.
If you are under 18 years of age, you must have a parent or legal guardian sign this form. You can’t sign it yourself. With your artwork, please include the signed form, which may be downloaded here: Copyright Permission Form.
Also, for the Heard Guild to send any payments to youth artists, the IRS requires that you complete this Federal Form W9 which can be downloaded here: W9.
No payments can be made to you until the Guild has these two forms so don’t forget them!
